【摘要】 目的 探讨多小切口减张法在乳癌根治术中避免植皮,预防皮瓣坏死的应用。方法 对56 例乳癌根治术中采用多小切口减张法预防术后皮瓣坏死的临床观察。结果 56 例乳癌根治术后其中有1例出现皮瓣坏死,但面积较小,经加强换药后1.5个月痊愈,其中2 例腋窝积液,经穿刺抽液、加压包扎后治愈。结论 多小切口减张法可有效降低乳癌根治术后皮瓣坏死的发生率,避免了植皮,减少创伤。

【Abstract】 Objective To discuss the effectiveness of milti mini incisions in avoiding skingrafting and preventing skin flap necrosis for patients received radical mastectomy.Methods 86 patients received radical mastectomy were enrolled to observe the effectiveness of milti mini incisions to relax skin tension.Results Small area skin flap necrosis was seen in 1 patient,subcutaneous fluid accumulation in armpit were seen in 2 patients,all patients were cured by local treatment.Conclusion Multi mini incisions can effectively lower the incidence of skin flap necrosis and avoid skingrafting.

乳腺癌仍是世界范围内最常见的女性恶性肿瘤,是癌症相关病死的主要原因[1]。乳腺癌发病率在我国占各种恶性肿瘤的7%～10%,近年来呈上升趋势,在部分大中城市已上升为女性发病率最高的恶性肿瘤。乳癌的治疗以手术根治切除为主,皮瓣坏死作为乳癌根治术后常见的并发症,一旦发生,将推迟术后综合治疗时间,影响综合治疗效果,增加了患者的住院时间和经济负担,而且常规的植皮增加了患者的痛苦和创伤。为减少皮瓣坏死的发生,2003年6月至2009年7月,我院对86例乳腺癌患者在根治术中采用多小切口减张法预防皮瓣坏死,临床效果满意,现报告如下。
1 资料与方法
