【摘要】 目的：探讨胃癌肝转移与多种危险因素之间的关系。方法：收集206例胃癌病例资料，选取12项指标作为胃癌肝转移的危险因素，采用Logistic回归法进行单因素和多因素分析。结果：单因素和多因素分析结果显示胃癌的浸润深度、浆膜侵犯、淋巴结转移和临床病理分期与肝转移有关，而其他因素则与肝转移无关。结论：胃癌肝转移与浸润深度、浆膜侵犯、淋巴结转移和临床病理分期有关。

Analysis of Risk Factors in Hepatic Metastasis of Gastric Carcinoma

【Abstract】ObjectiveTo investigate the relationship of hepatic metastasis in gastric carcinoma with different risk factors.MethodsClinical data of 206 gastric carcinoma patients were collected. Taking 12 data as the risk factors in hepatic metastasis, the correlation analysis including singular and multiple factors was conducted with logistic regression method.ResultsAs revealed by singular and multiple factor analysis, hepatic metastasis was related to infiltration depth, serosa invasion, lymph node metastasis and clinicopathologic stage. Other factors had nothing to do with hepatic metastasis.Conclusion The primary tumor involving the infiltration depth, serosa invasion ,lymph node metastasis and clinicopathologic stage of gastric carcinoma are risk factors related to hepatic metastasis.

肝脏是胃癌最常见的转移部位之一，肝转移也是影响胃癌患者预后的重要因素。因目前尚缺乏有效的手段对其发生进行预测，故患者的预后并未明显改善。
为了评估胃癌发生肝转移的危险性，本文采用Logistic回归法对可能与肝转移有关的多种危险因素进行单因素和多因素分析，旨在揭示其相关关系，为胃癌肝转移的早期预测和改善患者预后提供帮助。
1材料与方法
1.1临床资料
收集青州市人民医院2001年1月至2008年12月手术治疗并有术后组织病理学确诊的206例胃癌患者的住院及随访资料，其中男142例，女64例，年龄32~80岁，中位年龄56岁。随访日期截至2010年12月，其中有40例术后发生了肝转移。
1.2危险因素选择
选择性别、年龄、肿瘤部位、浆膜侵犯、肿瘤大小、浸润深度、组织学分型、分化程度、淋巴结转移、临床病理分期、CEA、CA199等12项指标作为胃癌肝转移可能的危险因素。危险因素选择与赋值方法见表1。

1.3统计学处理
