脑电图检查对什么诊断最有价值_动态脑电图对癫痫的诊断价值

  [摘要] 目的:探讨动态脑电图(AEEG)对癫痫的诊断价值。方法:对96例癫痫患者和78例非癫痫发作性疾病患者进行常规脑电图(REEG)和AEEG进行对比研究。结果:癫痫组REEG和AEEG的总异常率差异有统计学意义(P<0.01);癫痫组与非癫痫组AEEG的总异常率差异有统计学意义(P<0.01);清醒期共出现痫样放电29例(72.5%),睡眠期共出现38例(95.0%),二者比较差异有统计学意义(P<0.01),睡眠期痫样放电多于清醒期。结论:AEEG对癫痫的诊断和鉴别诊断有重要意义。

  The diagnostic value of AEEG to epilepsy

  [Abstract] Objective: To explore the diagnostic value of AEEG to epilepsy. Methods: The contrast study of routine electroencephalography (REEG) and ambulatory electroencephalography (AEEG) of 96 patients with epilepsy and 78 patients with non-epileptic attack disorder were conducted. Results: The total abnormal rate of REEG and AEEG in group epilepsy had statistical difference (P<0.01). The total abnormal rate of AEEG between epilepsy andnon-epileptic attack disorder had statistical difference (P<0.01). 29 cases (72.5%) occurred epileptic discharge in awake period and there were 38 cases in sleep period (95.0%), it had significant difference (P   动态脑电图是目前癫痫的辅助诊断及治疗评价最为有效的手段。动态脑电图对某些神经性症状的鉴别诊断尤为重要,如晕厥、发作性头痛、头昏,可用以鉴别其病源。对于心理原因引起的病症,动态脑电图可确认其心理因素与疾病发作的联系,以明确诊断。临床上动态脑电图可用于脑死亡的判断,此对脏器的移植具有重要意义,还可对颅脑外伤和昏迷及呼吸暂停综合征患者的动态监测,协助判断预后[6-7]。本组资料显示,非癫痫发作性疾病患者AEEG的总异常率为20.5%,且发作间期于睡眠中易出现脑电活动异常,无论临床表现还是脑电变化都提示癫痫有夜间发作倾向。说明虽然癫痫是一种反复性、发作性疾病,但并非在发作性的症状都是癫痫,AEEG可用于癫痫的鉴别诊断。
