血清乳酸脱氢酶偏高的原因【急性颅脑损伤患者血清中C反应蛋白及乳酸脱氢酶的表达及关系的研究】

  [摘要] 目的 检测急性颅脑损伤患者血清中C反应蛋白(CRP)及乳酸脱氢酶的表达量,探讨二者在颅脑损伤中的临床意义。方法 收集121例急性颅脑损伤患者和50例正常成人的临床资料,检测血清中CRP及乳酸脱氢酶的含量,分析其在不同临床特征及预后表达的关系。结果 急性颅脑损伤患者血清中CRP及乳酸脱氢酶含量升高,重型颅脑损伤中的表达明显高于非重型颅脑损伤患者,死亡组患者的表达明显高于存活组。结论 急性颅脑损伤时血清中CRP及乳酸脱氢酶含量升高,在颅脑损伤急性期联合检测血清中二者的含量对判断病情程度及预后有重要价值。

  Expression of C-reactive Protein and Lactate Dehydrogenase in Acute Traumatic Brain Injury and Its Clinical Significance

  [Abstract] ObjectiveTo detect the expression levels of serum C-reactive protein(CRP)and lactate dehydrogenase in acute traumatic brain injury and to explore the clinical significance of both in the injury. MethodsThe experiment included 121 patients with acute traumatic brain injury as experimental group and 50 adults as control group. The serum CRP and lactate dehydrogenase levels were detected in the two groups,and their clinical significance and relationship were analyzed. ResultsThe levels of serum CRP and lactate dehydrogenase were higher in the experimental group than those of the control group,,their expression levels in severe traumatic brain injury was significantly higher than those in non-severe traumatic brain injury and their expression in the death patients was significantly higher than that in the survival ones. ConclusionThe serum CRP and lactate dehydrogenase levels are increased in acute traumatic brain injury and it is of great value to detect both serum CRP level and lactate dehydrogenase level in the acute phase of traumatic brain injury in predicting the severity and prognosis of the disease.

  颅脑损伤是临床上常见的创伤之一,神经外科医师能准确判断颅脑损伤的严重程度,对于合理救治至关重要。传统上对急性颅脑损伤及预后的判断采用GCS昏迷评分法[1],近年随着检测技术的进步,血清中细胞因子的表达逐渐被用于早期诊断。本文通过对121例急性颅脑损伤患者血清中C反应蛋白及乳酸脱氢酶的含量进行检测,分析其临床价值,旨在为临床合理诊断治疗提供理论依据。
  1 资料和方法
  1.1 临床资料
  收集我院2008年1~12月急性颅脑损伤的住院患者,共121例,临床资料完整。其中男性87例,女性34例;年龄18~76岁,平均44.1岁。本组患者GCS评分为3~15分,其中3~8分21例,9~12分35例,13~15分65例。本组均排除糖尿病史、伤前饮酒史及有中枢神经系统疾病的患者;同时选取同期于我院体检时预留的成人血清标本50例作为对照组。
  1.2 方法
  患者入院确诊后,于次日早晨抽取空腹的静脉血3mL,使用我院检验科生化分析仪检测乳酸脱氢酶。CRP的检测采用免疫乳胶比浊法,检测均由同一检验师操作,步骤严格按照操作说明进行,严格质控。
  1.3 统计学方法
  数据均经Excel表格整理,以均数±标准差(χ±s)表示,采用SAS统计分析软件进行统计分析,两组间比较采用t检验,P   总之,急性颅脑损伤时血清中CRP及乳酸脱氢酶含量增高,在颅脑损伤早期检测CRP及乳酸脱氢酶对初步判断患者的预后可能有重要参考价值。
