摘 要 目的：探讨重症监护病房ICU 医院感染的临床特点，以采取相应的有效防治措施。方法：采取前瞻性与回顾性调查相结合的方法，对355例医院感染病例进行统计分析。结果：患者医院感染率为18.31％ ，显著高于同期医院平均感染率3.22％ ；医院感染部位的分布以下呼吸道感染最多，占64.50％；医院感染的病原菌以革兰氏阴性杆菌为主。结论：ICU是医院感染的高发科室，应作为重点监测部门；其收治的患者基础病严重、侵入性操作多、大量使用抗生素、老年患者免疫功能低下等均是导致发生医院感染的易感因素；积极采取有效的综合防范措施，是降低其医院感染发生率的关键。

AbstractOBJECTIVETo investigate the clinical feature of nosocomial infections in intensive care unit (ICU) ,based on which effective methods are taken. METHODSUse prospective monitoring and retrospective investigation to analyze the 355nosocomial infections patients. RESULTSIncidence rate of NI in patients in ICU was 18.31％ . It was significantly higher than average incidence rate of NI of the whole hospital at the same time. The lower respiratory tract was the most vulnerable site to infection, accounting for 64.50％of all hospital infection. The most common infection microorganism in ICU was Gram-negative bacteria. CONCLUSIONSThe incidence rate of NI in ICU is much higher than that in other departments. ICU should be treated as focal point of NI in hospitals. Risk factors depend on the severity of underlying diseases,invasive procedure, the quality of disinfection and sterilization, and patients immunity status especially in the old. The key to reducing incidence rate of NI is to take complebensive measures.

重症监护病房（ICU）的创建和完善对提高危重患者的抢救成功率起到了至关重要的作用，但感染性并发症也随之而来，感染可以导致抢救最终失败，因此控制和预防ICU医院感染十分重要。为探讨ICU患者医院感染的发病特点及危险因素，为控制和减少医院感染提供依据，对2006年1月～2008年6月ICU病房收治的355例患者中发生医院感染的65例进行分析，现报道如下：
1资料与方法
1.1临床资料
2006年1月～2008年6月我院ICU病房共收治355例患者，证实为医院感染65例，92例次。其中男40例，女25例，年龄在10～83岁之间，平均58岁，其中 ≥60岁67例占72.83 ％。住院天数6～109d，平均35.76 d.
1.2方法
采用前瞻性监测与回顾性调查结合的方法, 依照卫生部《医院感染诊断标准（试行）》,进行资料收集与分析。
