【摘要】 目的 探讨IL�8、TNF�α在COPD不同时期的变化和临床意义。方法 选择48例COPD急性加重期患者分为非机械通气组与机械通气组，经治疗后为缓解期组与顺利脱机组，分别检测四组血清IL�8、TNF�α浓度进行比较分析。结果 机械通气组IL�8、TNF�α水平在COPD急性发作期明显高于非机械通气组，经治疗后二者水平明显下降，但顺利脱机组仍明显高于缓解期组。结论 IL�8、TNF�α参与了COPD气道炎症过程，动态检测COPD急性加重期患者血清IL�8、TNF�α水平有助于判断病情的严重程度及预后。�

【Abstract】 Objective
To study the variation and clinical significance of IL�8, TNF�α with different stages in COPD patients. Methods 48 patients with acute exacerbation of COPD were divided to two groups, non� mechanical ventilation group and mechanical ventilation group, and relief group,successful offline group after their trearments, the serum levels of IL�8, TNF�αwere detected and compared in four groups. Results The levels of serum IL�8, TNF�α rose obviously in patients with acute exacerbation of COPD，The levels of mechanical ventilation group were higher than that of non� mechanical ventilation group, and significantly descended after their trearments. The levels in successful offline group were obviously higher than that in relief group. Conclusion IL�8, TNF�α have taken part in the COPD airway inflammation, whose levels have good effect on determining the condition and prognosis in patients with acute exacerbation of COPD.�

反复感染是加重慢性阻塞性肺疾病(COPD)气道炎症的最主要原因之一，目前普遍认为炎症细胞及其细胞因子在COPD气道炎症过程中起着重要作用。本研究通过对不同病期的COPD患者血清中IL�8、TNF�α水平的检测，探讨细胞因子在COPD病程中的作用和临床意义。
1 资料与方法
1.1 一般资料 选取2009年1月至2010年6月收住我科的COPD急性加重期(AECOPD)患者48例，入选标准符合中华医学会呼吸病学会《慢性阻塞性肺疾病诊治指南》诊断标准��［1］�，入院前2周内未用糖皮质激素。根据病情不同将其分为非机械通气组25例，经治疗后为缓解期组，男12例，女13例，年龄58~78岁，平均65.4岁；机械通气组23例，经治疗后均顺利脱机为顺利脱机组，男12例，女11例，年龄52~80岁，平均64.8岁；所有受试对象年龄、性别构成均无显著性差异(P
