【摘要】 目的了解影响肾移植患者监护期睡眠质量的相关因素和护理。方法 2004年至2006年150例肾移植患者监护期的睡眠质量情况,采用自行设计的睡眠因素调查表对患者调查相关的因素。结果 调查发现86%的肾移植患者监护期存在睡眠障碍,相关因素依序为心理负担、环境改变、身体不适、抗排斥药的副作用、经济负担。结论 做好肾移植患者监护期的心理护理、改善病区环境、增加舒适对提高患者的睡眠质量具有十分重要的意义。

The influencing factors and nursing on the sleeping quality in intensive-care renal transplant patients

【Abstract】 Objective To understand the influencing factors and nursing on the sleeping quality in intensive-care renal transplant patients.Methods The sleeping quality of 150 cases of intensive-care renal transplant patients was investigated by using a self-designed questionnaire from 2004 to 2006.Results Sleep disorders were found in 129 cases of patients(129/150 cases,86%),influencing factors in proper order were psychological burden,environmental change,physical discomfort,the side effects of anti-rejection drugs and economic burden.Conclusion Well psychological care,improving the ward environment and comfort are of great significance to improve the sleeping quality in intensive-care renal transplant patients.

肾移植是治疗终末期肾病的一个最有效的治疗手段。因肾移植手术的特殊性和术后并发症的高发性,需对术后患者进行5~14 d的隔离监护,而保证肾移植患者监护期的睡眠质量,对于预防术后各种并发症的发生、促进患者早日康复,具有十分重要的意义。2004~2006年,笔者对150例肾移植监护期患者进行睡眠质量及相关因素的调查,并实施护理干预,现报告如下。
1 资料和方法
1.1 一般资料 本组病例150例,男101例,女49例,年龄13~69岁,150例患者均在硬外麻下行同种异体肾移植术,术后回监护病区,行5~14 d的保护性隔离及监护,术后均采用他克莫司+吗替麦考酚酯+泼尼松(骁悉)三联抗排斥治疗,调查对象均能详细表达自己的主观感受[1]。
1.2 调查工具 采用临床观察法和自行设计影响睡眠因素调查表,评估患者睡眠质量,对患者的睡眠质量和影响因素进行调查。
1.3 调查方法 在对患者监护期的护理中,通过观察法具体了解患者的睡眠情况与用自行设计影响睡眠因素调查表了解影响睡眠的因素。调查内容包括患者一般情况、环境、疾病、心理、疼痛、经济负担等6个小项目,每个项目有影响和无影响两个选项,请患者在符合自己的实际情况的选项上打“√”,共发放150份,全部有效回收。
2 结果
2.1 睡眠障碍者129例,占86%,其中失眠者50例,表现为入睡困难,睡眠时间过少,24 h的总睡眠时间
