【摘要】 目的 探讨外伤性白内障摘除术后不同时期植入人工晶体的眼底病发生率的差别。方法 外伤性白内障患者110例(112只眼)全部行后房型人工晶体植入,其中Ⅰ期植入81例(82眼),Ⅱ期植入29例(30眼)。结果 Ⅰ期植入人工晶体术后视力在0.3以上71眼(86.59%),0.5以上27眼(32.93%)。Ⅱ期植入人工晶体术后视力在0.3以上26眼(86.67%),0.5以上6只眼(20.00%)。Ⅱ期植入人工晶体在术中并发眼内出血及术后人工晶体移位发生率方面明显高于Ⅰ期植入术。结论 Ⅰ期和Ⅱ期植入人工晶体均能提高术后视力, Ⅰ期植入人工晶体，术中出血发生率低，术后人工晶体偏离中心发生率小。Ⅱ期植入术后炎症反应轻,术中容易并发性出血，术后易发生人工晶体移位。

【Abstract】 Objective To explore the difference of Retina incidence rate caused by intraocular lens implantation in different periods after Traumatic cataractopiesis.Methods 110 patients with traumatic cataract (112 eyes) of all rows of posterior chamber intraocular lens implantation, implantation Ⅰ, 81 cases (82 eyes), Ⅱ implantation in 29 cases (30 eyes). Results Ⅰ intraocular lens implantation in postoperative visual acuity of 0.3 or more in 71 eyes (86.59%), more than 0.5 in 27 eyes (32.93%). Ⅱ intraocular lens implantation in postoperative visual acuity of 0.3 or more in 26 eyes (86.67%), more than 0.5 in 6 eyes (20.00%). Ⅱ intraocular lens implantation in complicated surgery and postoperative intraocular hemorrhage intraocular lens shift the incidence was significantly higher than in stage Ⅰ implantation. Conclusion Ⅰ and Ⅱ intraocular lens implantation could improve visual acuity, Ⅰ intraocular lens implantation, a low incidence of bleeding and postoperative incidence of intraocular lens off-center in small. Ⅱ of inflammation after implantation of light, easy concurrency intraoperative bleeding, postoperative intraocular lens prone to shift.

外伤性白内障是临床眼科常见病, 各种不同类型的眼外伤病情复杂, 常伴有眼球其他组织的损伤,特别是穿孔性眼外伤,炎症反应较重，多手术难度大,效果差,预后不确定［1］。白内障手术后Ⅰ期植入还是Ⅱ期植入人工晶体是临床上值得讨的问题。现回顾性研究我科2006年3月至2008年3月收治的110例外伤性白内障术后不同时期人工晶体植入患者临床资料,现报告如下。
1 临床资料
1.1 一般情况 我院2006年3月至2008年3月共收治的110例外伤性白内障患者110例患者共112只眼,其中男77例79只眼,女33例33只眼。年龄最小4岁，最大72岁,平均（32±10.5）岁。眼球穿通伤77例77只眼;钝挫伤21例21只眼,爆炸伤12例14只眼。同时伴有球内异物27例29只眼。
1.2 手术方法 儿童采用全麻+局麻,成人采用局麻,所有手术均在显微镜下操作, Ⅰ期植入人工晶体81例82只眼,囊袋内植入44例44只眼,睫状沟植入28例29只眼,睫状沟缝线固定9例9只眼。Ⅱ期植入人工晶体29例30只眼, 30只眼角巩膜裂伤修复术后1~2个月行Ⅱ期人工晶体植入术。睫状沟18例18只眼, 囊袋内植入8例9只眼,睫状沟缝线固定3例3只眼。其中行前部玻切15例15只眼,联合球内异物取出18只眼。
1.3 人工晶体的选择 如果创口未愈合,通过询问病史,了解患者双眼视力情况,通过对健眼测量的数据作伤眼参考依
据。如果创口愈合，可通过直接测伤眼角膜曲率、眼球长度等数值,使用SRK-Ⅱ回归公式计算所需要人工晶体度数。
1.4 术后处理 根据术前外伤和术后炎症反应情况,使用抗生素眼药水及皮质类固醇激素眼药水静脉滴注,结膜下或半球后注射地塞米松3 mg+庆大毒素2万U,典必珠眼液视情况使用,复方托品酰胺活动瞳孔。

使用SPSS 16.0统计学软件统计分析，采用卡方检验分析治疗前后眼压和视力变化，P
