【摘要】 目的 探讨亚低温脑保护疗法对救治重型颅脑损伤的疗效。方法 将132例重型颅脑损伤随机平均分为治疗组与对照组，进行对比分析，并且前瞻性分析两组疗效。结果 治疗组66例患者，存活率为77.3%，恢复良好率54.5%(36/66)，病死率18.2%(12/66)。疗效明显优于对照组，未发生相关的并发症。结论 亚低温治疗能显著降低重型颅脑损伤患者的死亡率，改善颅脑损伤患者神经功能预后，并具有安全、显著降低病死率及癫痫发生率等优点。

The clinical study of hypothermia treatment of severe craniocerebral injury

【Abstract】 Objective To study the brain protection of mild hypothermia therapy on the efficacy of treatment of severe head injury. Methods 132 patients with severe traumatic brain injury were randomly divided into treatment group and control group were analyzed, and prospective analysis of two groups.66 patients treated group, the survival rate was 77.3%, good recovery rate of 54.5% (36/66), mortality rate was 18.2% (12/66). The effect was better intreatment group than the control group, without any related complications. Conclusion Mild hypothermia can significantly reduce mortality in patients with severe traumatic brain injury and improve neurological outcome in patients with traumatic brain injury and is safe and significantly reduce the mortality and incidence of epilepsy and other advantages.

我院自2000年1月至2005年12月采用亚低温治疗重型颅脑损伤66例，临床疗效显著，现结合文献分析报告如下。
1 资料与方法
1.1 一般资料 治疗组66例患者均为伤后24 h以内的重型颅脑损伤(GCS≤8分)，伴颅内高压。同时选择66例为对照组。所有患者均经CT确诊，并排除其他脏器损伤引起的严重休克、缺氧或窒息；排除孕妇、14岁以下儿童及70岁以上老年人。两组患者主要临床资料差异无统计学意义（P>0.05），具有可比性，见表1。
1.2 治疗方法 治疗组：患者急诊入院或手术后立即采用亚低温治疗仪，对头颈部及全身进行有效降温，一般使用2~3 h后腋温即可降到37℃以下。物理降温同时给予：冬眠合剂(氯丙嗪50 mg+异丙嗪50 mg+度冷丁50 mg，肌内注射)。根据患者的体温、心率、血压和肌张力调整用量，保证患者安静、无寒战和收缩压不低于90 mm Hg为度。通常降温4~5 d，当患者颅内压正常时停亚低温药，撤去亚低温治疗仪自然复温。对照组：除不采用亚低温措施外，其余治疗及抢救措施与亚低温组相同。
1.3 监测指标 患者在入院或手术后即行以下各项指标监测：腋温、心率、呼吸、血压、血氧饱和度(采用多参数监护仪监测)。
