[摘要] 目的:探讨心力衰竭患者血浆B型钠尿肽(BNP)水平的检测及临床意义。方法:采用快速免疫荧光法分别测定本院2009年1～8月收治的54例心力衰竭患者及25例同期健康者的血浆BNP水平,对两组进行比较分析。结果:心力衰竭患者的血浆BNP水平明显高于健康者。结论:血浆BNP的水平对心力衰竭患者的诊断及严重程度评估有重要的临床价值。

The detection of plasma BNP levels in the patients with heart failure

[Abstract] Objective: To investigate the plasma BNP levels in the patients with heart failure. Methods: From January to August 2009 in our hospital, the plasma BNP levels of 54 patients with heart failure and 25 healthy subjects during the same period and healthy people over the same period were determined. The results were compared and analyzed. Results: The plasma BNP levels of heart failure patients was significantly higher than that of the healthy subjects. Conclusion: Plasma BNP levels has important clinical value for the diagnosis and severity assessment of heart failure patients.

B型尿钠肽(B-natriuretic peptide,BNP)是由心脏分泌的短肽激素,也叫脑尿钠肽,其具有重要的病理生理学意义,可以促进排钠、排尿,具较强的舒张血管作用,可对抗肾素-血管紧张素-醛固酮系统(RAAS)的缩血管作用,同心房钠尿肽(ANP)一样是人体抵御容量负荷过重及高血压的一个主要内分泌系统[1]。心功能障碍能够极大地激活利钠肽系统,心室负荷增加导致BNP释放。本研究主要探讨心力衰竭患者血浆BNP水平的检测,现将相关资料进行报道,以供参考。
1 资料与方法
1.1 一般资料
选择本院2009年1～8月收治的54例心力衰竭患者,均符合第5版《心脏病学》中心力衰竭的诊断标准[2]。其中,男30例,女24例;年龄43～68岁,平均(54.3±3.6)岁;心功能按按照美国纽约心脏病协会(NYHA)分级,Ⅰ级10例、Ⅱ级19例、Ⅲ级15例、Ⅳ级10例。选择同期体检的25例健康者作为对照组,其中,男16例,女9例;年龄42～70岁,平均(54.5±3.7)岁;均经临床查体、心电图、胸片、超声心动图、实验室检查排除器质性疾患。两组患者的一般资料间相比,差异无统计学意义(P>0.05),具有可比性。
1.2 方法
心力衰竭患者抽取静脉血3 ml用EDTA抗凝,健康者清晨空腹采集静脉血3 ml用EDTA抗凝,均采用Biosite公司的BNP-TRIAGE床边快速定量检测仪检测血浆BNP水平。
