【摘要】 阿利吉仑(aliskiren)是由诺华制药公司首创的新型口服低分子量非肽类肾素抑制剂,它作用于RAS的第一限速步骤,不同于ACEI及ARB;它不含肽链不引起含肽链的其他肾素抑制剂所产生的不良反应。是首个合成的新型非肽类肾素抑制剂。是一种治疗肾素型高血压、安全、有效的口服新药。

阿利吉仑(aliskiren)是由诺华制药公司首创非肽类新型口服低分子量非肽类肾素抑制剂,经临床研究具有良好的降压作用(对肾素型高血压降压效果更好),已成为副作用少,用药安全的全新型抗治疗肾素型高血压的新药。它没有以往肾素抑制剂如雷米吉仑(remikiren),依那吉仑(enalkiren)等分子中延伸的肽链,因此也就没有以往肾素抑制剂的副作用。而阿利吉仑具有自己的游离碱其分子式:C��30�H��53�N�3O�6,以半延胡索酸盐形式存在。与许多的肽类肾素抑制剂相比,阿利吉仑具有更好的理化性质,亲水性强(pH=7.4时,logPo/w=2.45),水溶性好(pH=7.4时, 2.3 安全性及不良反应 阿利吉仑不产生反射性心动过速,不影响心功能;降压效果好,降压作用为肾素依赖性,因此大剂量给药只会延长作用时间,不会导致血压骤降。最常见的不良反应是疲乏、胃肠道反应、头痛、头晕等,这些不良反应的发生并无剂量依赖性增加。与ARB相似,阿利吉仑不良反应的发生率与安慰剂组相比无明显差别[14]。ACEI在抑制ACE的同时,导致这些物质降解为无生物活性的产物受到抑制,因此在治疗过程中会产生一些不良反应如咳嗽、血管神经性水肿等。与ACEI不同,阿利吉仑对肾素具有高度选择性,可部分解释阿利吉仑极少的不良事件发生率。服用阿利吉仑后对肾脏有无不良反应、对肝脏有无不良影响以及与P-450酶的关系等,有待大规模、多中心长期观察逐渐明确。
3 展望
阿利吉仑是较为理想的治疗高血压的药物,但由于时间短,可能有的副作用尚未被发现,同时也还不清楚是否能有效地降糖尿病、心衰、肾脏病、动脉粥样硬化的发病率、死亡率。2005年Pilz等[15]在动物实验研究中初步显示,阿利吉仑的对心脏、肾脏的保护作用不逊于ARB,但其作用究竟是强是弱、是否能优于ACEI和ARB,尚需等待大规模临床试验的结果。2006年Schmieder[16]检索到在国际上,已有一些学者研究阿利吉仑对靶器官损害及相关临床情况干预疗效的临床试验,这些临床试验可能在今后2年内将陆续公布试验结果:其中包括观察左室肥厚的ALLAY研究(ALiskiren inLeft ventriculAr hypertrophy)、观察心衰治疗效果的ALOFT研究(ALiskiren Observation of heart FailureTreatment)以及观察对糖尿病蛋白尿疗效的AVOID研究(Aliskiren in the evaluation of prOteinuria InDiabetes)等。另外心血管事件1级、2级预防的临床试验以及心肌梗死后联合用药也将相继展开。展望将来阿利吉仑成为一线抗高血压药前景看好。
