【摘要】 目的 探讨湘西地区脂肪肝的发生率及与血脂、转氨酶水平之间的关系。方法 2008~2009年本地区企事业工作人员及个人体检者共计5 652例,用全自动生化分析仪测定,转氨酶采用速率法,血脂采用酶法,同时指定专人进行彩色超声波检查,诊断脂肪肝标准严格参照《超声医学》。结果 共检出脂肪肝1 974例,男1 755例,女219例,脂肪肝组的TG、ALT显著高于非脂肪肝组,且同时伴有高LDL-C、及低HDL-C。结论 脂肪肝组具有显著的血脂代谢紊乱,肝细胞损伤明显,其好发年龄在30~60岁之间,男性高于女性。

Exploration on the relationship between the incidence rate of fatty liver and blood lipid,aminotransferase level in Autonomous Prefecture of Xinangxi

【Abstract】 Objective To explore the relationship between the incidence rate of fatty liverand blood lipid, aminotransferase level in Autonomous Prefecture of Xinangxi. Methods Specimens supplied by 5 652 cases including staffs of enterprises and institutions and individuals in Xiangxi from 2008 to 2009,which were detected by full automatic biochemical analyser. Aminotransferase and blood lipid were respectively adopted by enzymatic methods and kinetic methods.Meanwhile, specific persons were designated to take charge of examinations by Color Doppler Ultrasonograthy (CDU). The standard of diagnosing fatty liver strictly refered to medical ultrasonics as a reference. Results It was found that 1974 patients with fatty liver,male 1755 cases, female 219 cases. The level of TG and ACT in fatty liver groups is not only much higher than non-fatty liver groups but also with high LDL-C and low HDL-C. Conclusion Fatty liver groups revealed highly blood lipid metabolic disturbance and liver cells were seriously injured.Ages of the incidence rate range from 30 to 60 and the cases of the male is more than the female.

随着人民生活水平的不断提高,饮食结构营养丰盛化,脂肪肝的患病率在逐年上升。脂肪肝发展为肝硬化、肝癌及肝功能衰竭的机率远高于健康者。湖南省湘西地区是一个以土家族和苗族为主的少数民族居集地,为了解本地区人群脂肪肝的发生率与血脂、转氨酶水平的关系,现将2008~2009年来本院行健康体检人员资料分析如下。
1 资料与方法
1.1 一般资料 5 652例均来自2008年1月至2009年10月来我院体检科进行健康体检的各企、事业在职工作人员及部分离退休人员及个人体检者。年龄20~80岁,其中男3 690名,女1 962名,本统计资料包括:空腹肝、胆彩超检查及血脂、转氨酶检测。
1.2 方法
1.2.1 仪器及试剂 Olympus 640全自动生化分析仪,试剂由日本第一化学药品株式会社提供,每天用英国朗道公司提供复合标、高、低质控物随标本进行定标和室内质控.转氨酶采用速率法,血脂采用酶法,严格按临床操作规程[1]进行测定。
1.2.2 操作 严格按体检要求,保持平常的生活和饮食习惯,3 d前忌酒,空腹12 h后抽取静脉血,置室温半小时后分离血清,当天测定完备,测定项目包括:ALT、AST、TC、TG、HDL-C、LDL-C。
1.2.3 超声诊断脂肪肝标准参照≤超声医学≥[2],脂肪肝诊断标准。
1.3 统计学方法 数据采用SPSS 11.5软件进行统计学分析,正态分布资料以均数±标准差(x±s)表示,组间比较采用t检验,计数资料比较用χ�2检验,结果用百分比率表示(%),P0.05)。
