【摘要】 目的 了解衡阳市蒸湘区社区医院医患关系的现状,探讨影响医患关系的原因和解决措施,为构建和谐的医患关系提供依据。方法 采用中心区拦截式调查方法,对衡阳市蒸湘区9所社区医院的医务人员和患者进行问卷调查。结果 该区医患关系总体趋于紧张;对医患关系紧张的责任,患方认为是医方的医术水平低、治疗效果差,而医方认为是患方的素质偏低、无端惹事生非;医方的医疗服务态度恶劣是目前医德医风中的突出问题;造成医患关系紧张的主要原因是药品价格过高、现行的医疗体制不合理、医疗保险制度不完善等;改善医患关系的主要措施有降低药品价格和检查费用、加强对医院和医务人员的财政投入、增加对患者的医疗补助和医疗保险等。结论 目前该区医患关系总体趋于紧张;建立和谐医患关系的主要措施有降低药品价格和检查费用、加强对医院和医务人员的财政投入、增加对患者的医疗补助和医疗保险等。

Research on doctor-patient relationship in community hospitals of zheng xiang district of Hengyang city

【Abstract】 Objective The aim was to introduce the actuality and the traits of doctor-patient relationship in community hospitals of Zheng Xiang District of Hengyang City, analyze the reasons why doctor-patient relationship nowadays is so strained in China and provide a scientific basis of constructing harmonious doctor-patient relationship.Methods Using the central area of block-style survey methodology to investigate the doctors and patients in 9 community hospitals.Results Doctor-patient relationship in community hospitals of Zheng Xiang District was strained.Patient believes that the primary responsibility of doctor was the medical effects of low levels of bad treatment, and the doctor believe that the patient suffering from the main responsibility was the low quality of trouble for no reason.The worst service attitude was the main problem of medical ethics.Tension caused by doctor-patient relationship is the main reason for high drug prices, the existing health care system is unreasonable and so on.The main measures for improving the doctor-patient relationship were reducing drug prices and inspection costs, strengthening financial investment, increasing medicaid patients and medical insurance and so on.Conclusion Doctor-patient relationships in community hospitals was strained.The main measures for establishmenting harmonious doctor-patient relationship were reducing drug prices and inspection costs, strengthening financial investment, increasing medicaid patients and medical insurance and so on.

近年来,我国医患关系日趋紧张,医疗矛盾日益突出,已成为严重的社会问题。医患矛盾给社会成员带来诸多的不便,扰乱了正常的社会秩序,与目前我国努力构建和谐社会的目标背道而驰。为寻求改善日益恶化的医患关系,本课题采用中心区拦截调查方法,对衡阳市蒸湘区9所社区医院的医患关系进行研究,分析该区医患关系的现状及其影响因素,为构建和谐的医患关系提供建议。
1 资料与方法
1.1 一般资料 衡阳市蒸湘区9所社区医院的医务人员(医生、护士、医技人员)和患者(住院患者、门诊患者)。
1.2 测评方法 采用自行设计的医患关系调查问卷进行调查。问卷内容涉及医患双方对医患关系的认知、医患交流和沟通、造成医患关系紧张的原因以及如何解决医患矛盾等。
1.3 抽样方法及样本量 采用中心区拦截问卷的调查方法进行调查,共调查240名医务人员和患者。
1.4 统计学方法 采用SPSS 13.0软件建立数据库,运用R×C行列表的卡方检验对调查结果进行统计分析。
2 结果
2.1 一般结果 对医方的问卷调查,发放问卷104份,回收问卷98份,有效问卷回收率94.23%;患方发放问卷155份,回收问卷142份,有效问卷回收率91.61%。调查对象中男占52.83%,女占47.17%;最小年龄15岁,最大年龄77岁,平均为(38.59±10.73)岁;文化程度构成中小学、中学、高中/中专、专科、本科、研究生各占5.42%、12.50%、14.17%、22.92%、34.16%、10.83%。
2.2 对医患关系的总体认识 医患双方大部分人认为目前医患关系有点儿紧张,经卡方检验,两组间分布的差异有统计学意义χ2=6.07,P 表1

2.3 医患关系紧张的责任
对医方所承担的最主要责任的调查中,有近40.00%的医务人员选择了“医院收取的医疗费用太高”,有近一半的患者选择了“医术水平低,治疗效果差”,经卡方检验,两组间的差异有统计学意义χ2=29.76,P
